Mobile Vaccine Clinics for Students
Northshore School District is partnering with Shoreline Fire Department and neighboring fire departments, along with UW Medicine to offer the Pfizer COVID-19 vaccination at middle and high schools for students ages 12+.

We are no longer accepting sign ups for the mobile vaccination clinics.
Vaccination clinic schedules are based on hybrid learning groups. If your student attends a school that is not listed, or is 100% remote, you will be able to choose the clinic that works best for your schedule. You must sign up by noon the day before your clinic. This gives Shoreline Fire Department enough time to prepare the correct amount of doses.

What to Expect
- Your student must bring the consent form signed by a parent/guardian to the clinic. While there are translations of the consent form below, students must bring the signed English copy to the clinic.

- You cannot receive a dose of the vaccine if you have been vaccinated with anything two weeks prior to your first shot.
- Parents and guardians will not be allowed on campus during the clinics. If families drive their student to a clinic, they will need to wait in their car while their student receives the vaccine.
- Transportation and additional support is available for students who receive Special Education services.

The second dose can cause people to feel unwell. Parents and guardians should plan to be available by phone following their student’s vaccination in the event that the student needs to be picked up from school.
